Abstract
Objective:To study the dynamic characteristics of peri-hematoma tissues on fMRI
Methods:The models of intracerebral hemor-rhage were setted up in the way of injecting fresh self-blood into the frontal lobes. 12 dogs with intracerebral hemorrhage underwent T2WI、DWI andPWI in 0.5 h,3 h,6 h,12 h,24 h,48 h,72 h,7d and 15 d.
Results:In the region of perihematoma, T2WI showed hyperintense vasogenic edema in
3 h. The periods of edema peak were in 48~72 h. The signal changes of perihematoma on DWIwere more complicated. ADC values elevated in theregions of focal hyperintense,which reflected vasogenic edema,whereas ADC values decreased in the regions of focal hypointense,which reflected is2chmia and cytotoxic edema. PWI showed hypo - perfution in 0.5~6 h. The hypo-perfution of perihematoma recovered and approached to the per-fution of contralateral region in 12 h. Some of cases showed hyper-perfution.
Conclusion:The dynamic fMRI has important applied values instudy ischemic injury and hemorrhagic edema of peri-hematoma cerebral tissues.

脑出血(intracerebral hemorrhage, ICH) 最重要的病理改变是血肿本身及其周围继发性病理损害—脑缺血和脑水肿。但脑出血后血肿灶周脑血流量( cerebralblood flow,CBF) 的下降是否会导致缺血性损害;血肿灶周是否存在缺血性半暗带(ischemia penumbral, IP) 和再灌注损害;血肿灶周缺血与脑水肿形成的关系,一直是目前争议的焦点[1,2 ] 。本研究利用功能性磁共振技术(functional magnatic resonance imaging,fMRI) 对血肿灶周病理变化进行探讨和研究。 |
