椎管内皮样囊肿和表皮样囊肿的MRI征像分析

ABSTRACT

  Object: to evaluate the characters of intraspinal Epidermoid and dermoid cysts on MRI.

Methods: the MRI imagings of 11 patients with Intraspinal  Epidermoid (n=9)and dermoid cysts(n=2) including 3 recurring cases as well,which were confirmed with operation and pathology, were anakyzed retrospectively. 
  Result:  All of the lesions were found in the lower lumbar region,locate at the conus and cauda equina,and had typical shape and well-defined border to the normal spinal on MRI.10 lesions showed heterogeneous signal intensity composed in iso-,hypo- and hyperintensity except that one lesion had iso-intensity on T1WI.All of the lesion were iso- to slightly hypointensity compared to CSF on T2WI.It also cound be found the iso- to hypointensity in the tumors on T2WI.No enhencement were detected in the cyst after adminstration of Gd-DTPA in the three cases.It was almost the similar charaters ad its original lesion in the three recurring tumor on MRI.
  
Conclusion:  MRI is powerful tool to assess the lesions of intrspinal epidermoid and dermoid cysts.

椎管内畸胎类肿瘤少见,主要是表皮样囊肿和皮样囊肿,发病率占椎管肿瘤的1%~2[1]; 畸胎瘤则十分罕见。此类肿瘤的诊断主要靠MRI。由于肿瘤的MRI表现复杂,加之认识不够,术前常不能作出准确的定性诊断。本研究总结分析了经手术病理证实的9例表皮样囊肿和2例皮样囊肿的MRI表现,并与手术病理进行了对照分析。
