评价实时三维多平面成像超声心动图测量左室容积准确性的体外模型研究

ABSTRACT

Objectives:This study was performed to assess the accuracy of real-time three-dimensional echocardiography (RT3DE) multiplane imaging for measuring left ventricular (LV) volumes in vitro.

Methods:Twenty left ventricular phantoms in different shapes were made of balloons (including 12 normal LV phantoms and 8 LV aneurysm phantoms). The volumes of these phantoms were quantified by RT3DE multiplane imaging and conventional 2DE, respectively. The results were compared with the corresponding actual volumes determined by volumetric cylinder.

Results:In both 12 normal LV phantoms and 8 LV aneurysm phantoms, the LV volumes measured by RT3DE multiplane imaging correlated quite well with the actual ones. The agreements were pretty good between these two methods. And there were no statistical differences between them. In the same phantoms, although there were also good correlations between the LV volumes measured by 2DE and the actual volumes, the agreements were bad between these two methods. And whether in normal LV phantoms or in LV aneurysm phantoms, the LV volumes measured by 2DE were statistically different from the actual volumes (P<0.05).

Conclusions:Compared with 2DE, RT3DE multiplane imaging can quantify left ventricular volumes rapidly and accurately, which established the ground of assessing left ventricular function reliably.

心室容积的精确测量是准确评价心室功能的基础。当前,二维超声心动图仍然是临床工作中应用最广泛的无创性测量心室容积的方法,但是在实际应用中,该方法却经常受到其对心室形态进行几何假设的限制,从而难以真实、准确地反映心室,尤其是某些疾病状态下(如心肌梗死后室壁瘤形成)心室的形态和容积[1]。尽管三维超声心动图能够有效地克服这一缺点,但是先前的多种测量方法过程复杂,技术要求较高,因此至今未广泛应用于临床[2]。如今,建立在实时三维超声心动图(Real-time three-dimensional echocardiography, RT3DE)基础上的多平面成像(Multiplane Imaging)技术能够实时、简便地测量心室容积,本研究应用体外模型,对这一新技术测量左心室容积的准确性进行评价。
