CT肝硬度分级评估肝储备功能的价值研究

Abstract

  Objective:Discussing the assessment value of CT grade for preoperative liver reserve function of liver cancer.

  Methods:Fifty nine hepatectomy cases have been studied for the patents who were all checked by two-phase contrast CT.The grade difference was analyzed by comparing preoperative CT grades with postoperative pathologic liver fiber grade,clinical liver function grade,preoperative and postoperative biochemical index change curve and survival rate.

  Result:The difference of survival rate between CT grade 1 and CT grade 2,3 was of valuable significance in statistics (P<0.05).The difference of peak value about ALT,AST and PAB between CT grade 1 and CT grade 2,3 in preoperative,postoperative and 20 days later was of valuable significance in statistics (P<0.05).The correlation of CT grades to pathology liver fiber grade was 0.77(P<0.0001).The correlation of CT grades to C-P grades was 0.46(P=0.0011).

  Conclusion:Liver CT grade can be one of important index of assessment liver reserve function of patients liver cancer before hepatectomy.

  肝硬化可以影响肝脏的储备功能,肝储备功能不足是术后导致肝衰竭的主要原因。而预防的关键就在于术前对肝癌患者肝功能能否耐受肝切除术做出评估[1]。本文试图通过对肝癌的背景肝脏,进行肝硬度CT分级,并与肝切除术后的生化改变、近期预后及病理所见进行对照研究,探讨肝硬度CT分级评估肝储备功能的价值。
